Download the pdf and then fill it with ball pen. Also this form needs to be certified by gas agency to which you are applying for LPG connection under the Ujjawala Yojana 2.0. Then in the online application form this form needs to be uploaded along with the required documents.
Who cannot apply for Ujjawala Yojana 2.0 LPG connection?
1) Should not possess a 2/3/4 wheeler vehicle.
2) Should not be tax payee.
3) Own Mechanized 3/4 - wheeler Agricultural
equipment.
4) Should Not Own 3/4 wheeler Agricultural equipment.
5) Possess Kisan credit card with credit limit of over 50,000/-.
6) Owns a refrigerator.
7) Household member is a government employee.
8) Owns landline phone.
9) Households with non-agricultural enterprises
registered with government.
10) Owns more than 2.5 acres of irrigated land with
1 irrigation equipment.
11) Do not have any member of household earning
more than 10,000 per month.
12) Should not have irrigated or owned a land of 5 acres for 2 or more crop seasons.
13) Paying income tax.
14) Should not be a land owner of more than 7.5 acres of agricultural land.

While uploading the form, it's asking for the document number? I don't see any number on the declaration form. Can you please suggest what needs to be entered?
